  • What is transition planning?

    Transition Planning is the process of services to assure that students with disabilities will be able to lead rewarding adult lives. Planning should begin during a students Freshman year.

    It takes place when parents, educators, adult service providers and the student sit down together to develop a Transition Plan as part of the Individual Education plan (IEP) process. The IEP should contain transition related goals and objectives that will reflect the student’s strengths, interests, needs, family values and a path to reach his/her goals for the future.

    Transition planning for individuals addresses:

    • present and future living arrangements
    • travel within the community
    • personal management skills
    • developing and maintaining relationships with the family and friends
    • medical service needs
    • advocacy and legal service needs
    • vocational training options
    • postsecondary education
